List current and prior addresses covering the previous 3 years.
List each unexpired commercial license or permit and licenses held during the past 3 years.
For each equipment type, provide equipment details, dates operated, and approximate total miles.
List employers for the last 3 years and, if you operated a CMV, the additional 7 years (10 years total). Explain every gap over one month. Include military experience where applicable.
